Access to Medical Records

The medical record is a vital component of the health care services provided to the patient as a means of communicating between clinicians as well as maintaining a chronology of all diagnostic tests, treatments, and care provided to our patients.  Active medical records are used daily to record and support medical decision making.  

Requests for copies of medical records can be made by completing and submitting the Medical Records Request Form below.  Once completed, the form should be sent to doc.medicalrecords@doc.ri.gov.
